The Questionnaire Tool
The Questionnaire tool provides a way for teachers to quickly develop online questionnaires and forms. The tool allows the teacher to create a questionnaire with short answer and fixed field responses. Teachers can choose the number of response options for the fixed field and can set these.
Special features include an option to allow students to see the responses of other students. Teachers can view the results at any time maling this a very useful research and feedback tool. The tool does not mark responses, it is not a testing tool (but can be used as one if you are preapred to do the marking)

Questionnaires as a learning activity
